This form will be used by the Tri Hamlet Leadership Council, a coalition of community groups, to assess the relief needs of residents. All information is confidential and will only be shared with agency officials participating in the Tri Hamlet Relief effort. Information will be used to refer residents to resources and aid that can be of assistance.
